Part of our practice policies includes filling out paperwork prior to scheduling your first appointment. Once completed, submit your files through the Secure Spruce App.

Schedule Your First Appointment
All patient appointments are via Telehealth through the Spruce App or desktop. You can schedule your appointment by calling our office or messaging through the secure Spruce App.
